Before you submit your rebate, please ensure that you have a Valid Email Address and a copy of your original Sales Receipt or Invoice, which shows:
- Proof of Purchase
- Customer Name and Address
- Date the tires were purchased
- Name and address of the store where the tires were purchased
- Tire product purchased
