How to redeem your rebate
- Requests must be postmarked (if mailed) or submitted online by December 30th, 2025, 11:59 EST. You may view and submit the online Rebate Form here.
- To redeem your mail-in rebate, complete this rebate form and send both pages along with proof of purchase (i.e. original sales receipt or satisfactory proof of purchase) dated between November 1, 2025 and November 30, 2025 to:
